Job Description
Job Summary/Overall Object i ves The Medication Adherence Specialist supports Independence Health System's value-based care initiatives by improving medication adherence performance across payer programs, including Medicare Advantage and Part D. This role focuses on identifying patients at risk for non-adherence, conducting outreach to address barriers to medication access and persistence, and coordinating adherence interventions in collaboration with pharmacies, providers, and internal care teams. The Medication Adherence Specialist plays a key operational role in closing medication adherence gaps that directly impact quality metrics, payer incentives, and patient outcomes. This position supports data driven performance improvement efforts through patient engagement, documentation, and coordination of refill and pharmacy services.
