Job Description

Farsi/English Bilingual Activity Coordinator Costa Mesa, CA 92627 Full-time Full-time Costa Mesa Adult Day Health Care Center is looking for a bilingual (Farsi/English) Activity Coordinator. Responsibility:
Planning and Operating Activity Programs Specific Duties:
1. Assess the needs and interests of each participant and develop an individualized activity plan as part of the individualized plan of care developed by the assessment team. The activity coordinator participates in the initial and continuing multidisciplinary team assessment and development of an individualized plan of care for each participant. 2. Provide signed and dated documentation for all services performed the day the service provided in each participant's record. 3. Maintain signed quarterly progress records and individualized activity plans in the participant's health record. 4. Develop, implement and supervise the activity program. Plan activities that address the needs of the participants, as specified in the individual plan of care and provide or supervise the daily activities. 5. Therapeutic activities organized by the CBAS center Activity Coordinator, which include group or individual activities to enhance social, physical, or cognitive functioning; facilitated participation in group or individual activities for CBAS beneficiaries whose physical frailty or cognitive function precludes them from independent participation in activities. 6. Involvement of participants in the planning of the program. 7. Post a schedule of planned activities in a central location readily accessible to participant, family, guardian or authorized representative. 8. Shall provide equipment and supplies necessary for the program, including special equipment and supplies necessary for participants having special needs. 9. Plan the use of volunteers and supervise their activities with the participants. Recruit, train and supervise volunteer activity aides and other volunteer guests from the community. 10. Arrange for transportation to community health related activities indicated by the participants' needs and interests. Search for community volunteers and programs, and plan and supervise transportation to and from activities outside of the center. 11. Activity Coordinator is a full-time position.
Qualifications:
The activity coordinator shall be knowledgeable of the physical, mental and social needs of the elderly and meet one of the following qualifications: 1. The activity coordinator shall have two years of experience in social, recreational or educational program within the past five years, one year of which was full-time employment in a patient activities program in a health care setting, mental health setting or setting for the care of the elderly, or 2. Shall be an occupational therapist, art therapist, music therapist, dance therapist, recreation therapist, occupational therapy assistant or a qualified social worker, or 3. Shall have completed at least 36 hours of training in a course designed specifically for this position and approved by the Department of Health Services and shall receive regular consultation from an occupational therapist, occupational therapy assistance, recreation therapist, art therapist, music therapist or dance therapist who has had one year of experience in a health care setting. 4. Be knowledgeable of all
ADHC/CBAS
laws, regulations, CBAS Certification Standards, CBAS Standards of Participation, and Policies and Procedures. This will be ensured through ongoing staff training and education. 5. Tuberculosis clearance and health examination performed not more than twelve (12) months prior to employment are required. 6. First aid and cardiopulmonary resuscitation training shall be provided to the employee within the first six (6) months of employment.
