Job Description

McGregor PACE (Program of All-Inclusive Care for the Elderly) is a community service program that provides in-home healthcare services to the elderly as an alternative to nursing home placement, allowing seniors to remain at home. We are looking for a Life Enrichment Coordinator who will be responsible for planning and implementing therapeutic recreational activity programs for participants.
Responsibilities:
Implements the coordinated, multi-faceted activity program designed by the Activities Director for frail, older adults, including those with mental illness or dementia, vision/hearing or physical impairments. With guidance from the Activities Director, will plan appropriate therapeutic activities and implement individual and group activities that accommodate the needs of the participants. Reports participant status changes to the Activities Director and other staff, as relevant. May be asked to perform initial and periodic participant assessments. Multidisciplinary team support includes completion of initial comprehensive assessments of participants by enrollment date: formal reassessments with reevaluation of the care plans that are to be completed as needed; assists in the coordination and documentation of a 24-hour care delivery for participants with input from all disciplines of the multidisciplinary team; initial and ongoing assessments of participants with a focus on physical, mental or emotional changes that are to be reported to the multidisciplinary team and reflected on the plan of care as needed. Communicates with participants and families regarding activity programming, calendar, and special events. Informs the Center Assistants and Volunteers of special activities. Each week, restock the Activity Cart/Activity Area with resources for independent leisure pursuits and small group activities that volunteers or Center Assistants can implement. Works with Activities Director to coordinate community involvement in activity programming (speakers, entertainers, etc.) Makes recommendations for purchasing activity supplies according to agency policy and procedure. Ensures compliance with PACE regulations. In collaboration with the Activities Director, assists with the coordination of other therapists, special programs as needed, including art, music, horticulture therapy. May be asked to assist with departmental and participant outcome data collection and analysis; participates in performance improvement initiatives, as relevant. Provides educational materials and professional presentations to agency staff, as relevant.
Qualifications:
High School Diploma required. A bachelor's degree in art, music, or recreational therapy is preferred. Certified Activity Coordinator preferred. Minimum two years of experience in a social, recreational or MultiCare facility providing and coordinating services for a frail or elderly population within the last five years, one of which was full-time in a patient activities program in a health care setting preferred. Experience working with dementia participants is desirable. Valid Ohio driver's license and working car required. Experience in a multidisciplinary care setting is desirable Computer literacy, typing skills, and formal presentation/public speaking experience preferred.
