Registered Nurse/Case Manager Job Summary:
We are seeking a dedicated and experienced Registered Nurse/Case Manager to join our healthcare team. The primary role of the RN/Case Manager is to oversee and coordinate patient care, ensuring that each patient receives the highest quality and most appropriate care possible. This individual will work closely with physicians, patients, and other healthcare providers to develop and implement comprehensive care plans.
Responsibilities and Duties:
1. Assess and evaluate patient needs and develop individualized care plans 2. Coordinate and communicate with physicians and other healthcare providers to ensure seamless patient care 3. Monitor and track patient progress and make appropriate adjustments to care plans as needed 4. Provide patient education and support to promote self-management and wellness 5. Collaborate with interdisciplinary team members to ensure continuity of care 6. Advocate for patients and their families to ensure their needs are met 7. Document patient assessments, care plans, and progress notes accurately and in a timely manner 8. Oversee and supervise other healthcare team members as needed 9. Participate in quality improvement initiatives and performance improvement projects 10. Stay current on best practices and evidence-based guidelines in nursing and case management
Qualifications and Skills:
1. Current Registered Nurse license in the state of practice 2. Bachelor's degree in Nursing (BSN) preferred 3. Certification in Case Management (CCM) preferred 4. Minimum of 2 years of experience in nursing, with at least 1 year in case management 5. Strong communication and interpersonal skills 6. Ability to work independently and as part of a team 7. Excellent organizational and time management skills 8. Knowledge of healthcare laws and regulations 9. Proficiency in electronic health record systems 10.
