Job Description
Overview Responsibilities Collaborates with ambulatory infusion suites, payers, physicians, clinicians, referral sources, home health agencies, and partner pharmacy/CarepathRX teams to facilitate patient discharge and coordination of care to the home.
Qualifications Minimum Qualifications:
Bachelor's degree in nursingLicensure, Registration, Certification:
Licensure, Registration, Certification One of the following:- Current Alabama RN license
- Current Multi State RN License in accordance with Nurse Licensure Compact (NLC) for Alabama
- Infirmary Health abides by the NLC requirements and guidelines for the state of
