Full Job Description Overview:

Responsibilities:
Coordinates care with referring clinicians and referral patients to simplify access and enhance communication and the patient/family experience at USA Health-Infectious Disease Clinic. Assesses educational needs in collaboration with all levels of nursing and medical personnel and evaluates the effectiveness and efficiency of educational programs, utilizing a systematic approach. Accesses to multiple network clinical information systems to gather and verify patient demographic and clinical information. Assists in identifying and documenting issues related to transitions of care and will round on patients as appropriate while monitoring their healthcare experience to ensure all expectations are being met. Provides individualized support, education and information, helping to make care seamless, continuous and comprehensive.
Facilitates patient Scheduling functions:
assists schedulers and coordinators in scheduling appointments, both new and follow up, and testing. Identifies, designs, and implements new strategies that will increase service utilization. Evaluates new markets, to include statistical analysis of potential markets, estimates of growth, identification of client needs, and competitive analysis Reviews patient case lists generated by technology-enabled data triggers to identify patients that could benefit from intervention, while tracking outcomes of referrals and treatments and maintaining detailed records that measure effectiveness of the program. Supports the roles and responsibilities of the Ryan White Grant Program managed under the Internal Medicine Infectious Disease Division of the Department of Internal Medicine. Supports in clinic needs for both ID clinic and Ryan White including patient intake, procedures, medication/injection administration, check out and any necessary follow up post visits Telephone and in-person triage for both ID and Ryan White Collaboration with ID providers and OPAT nurse to coordinate patient needs Conduct appointment referral request from providers as well as schedule/obtain/send any necessary needs for diagnostic testing for both ID and Ryan White Completes all mandatory department, educational and hospital requirements Adheres to current Infection Control and Safety Standards Regular and prompt attendance Ability to work schedule as defined and overtime as required Related duties as assigned
Additional Information:
Employees must be in a regular position, working 20 hours or more per week (.50 FTE or greater) to qualify for benefits.
Qualifications:
Graduation from an accredited school of nursing or practical nursing meeting the minimum standards set by the state of Alabama and 3 years of related experience Required LPN

  • State Licensure Required or RN
  • Registered Nurse
  • State Licensure and/or Compact State Licensure in accordance with the Nurse Licensure Compact (NLC) for Alabama Required Comparable combination of education and experience may substitute for the above requirements.
