Job Description

Nursing degree, active registered nurse (RN) license and minimum 1+ years Home Health RN experience required. Applicants who do not meet these qualifications will not be considered. Schedule:
Full-time, Monday through Friday On-call duties are probable Daily structured patient visit expectations
Qualifications:
Active RN license At least 1 year of recent home health RN experience Demonstrated proficiency with OASIS required Experience with HomeCare HomeBase is strongly preferred Capable of managing a structured, full-time caseload Reliable transportation and a valid driver's license
Key Responsibilities:
Provide skilled nursing care and interventions to patients in their homes Conduct comprehensive patient assessments and accurately complete OASIS documentation in a timely manner Collaborate with physicians, therapists, and the wider care team to create and implement individualized care plans Deliver wound care, medication management, disease monitoring, and patient/family education Maintain clear communication regarding patient status and updates with the clinical team
