Job Description

The Green House Cottages of Belle Meade is looking for a qualify candidate for our MDS/ Admissions Coordinating Nurse backup. The ideal candidate must have an active LPN or RN license , be able to direct the day-to-day functions of the MDS office in accordance with current rules, regulations, and guidelines that govern the long-term care facility. Participate in the implementation of the facility's Quality Assurance/Performance Improvement. Assist in developing methods for coordinating MDS services with other resident services to ensure the continuity of the resident's total regimen of care. Must be able to meet/exceed target census goals, meet with physicians, case managers, and other healthcare professionals to determine the needs of the patient. Must be able to review and convert referrals into admissions in a timely manner, facilitate admission paperwork, conduct tours of the facility, attend and host care plan or admission meetings. Medical assessments skills and computer skills are a must and knowledge of payer sources and State and Federal Regulations are preferred. Must be able to work in a fast paced environment and relate to medical professionals with a high degree of tact and poise, as well as have compassion and the ability to handle emotional situations. Must be able to pass a background check.
MDS Coordinator Job Description:
Conduct and coordinate the development and completion of the resident MDS according to CMS rules & regulations Complete medical forms and reports as directed Evaluate each resident's condition and pertinent medical data to determine any need for special assessment activities. Ensure that the care plan includes measurable objectives and timetables to meet the resident's medical, nursing, mental and psycho-social needs Ensure that an initial resident assessment is completed timely Ensure that all personnel providing direct care to the resident are aware of the plan of care and that they refer to the care plan prior to administering daily care Ensure that quarterly and annual MDS and care plan reviews are made on a timely basis.

Meet target goals and quality mix for facility or facilities assigned Establish monthly achievable targets to realign with company established goals Develop face-to-face business relationships with referral sources, elders and families, through continuous product/program education. Maintain accurate and updated PCC/CRM database Ability to facilitate MDS and admission paperwork Conduct tours of the facility Complete and maintain all required reports in a timely manner Knowledge of payer sources relevant to State and Federal Programs, along with facility contracts Knowledge of State and Federal regulations that pertain to MDS and admissions
Additional Job Demands:
Ability to work with individuals at all levels both within the organization and with outside contacts Knowledge of functions of office equipment; including computer skills with accurate typing ability Medical assessment skills to properly identify and convey medical condition and patient needs to facility Payer source knowledge required to estimate potential costs, insurance coverage, and financial arrangements for residents
Physical Effort:
Work is active; reaching, handling and finger dexterity required in operating machines (i.e., computer equipment, copier, fax, telephone, etc.). Walking and standing is necessary; lifting and/or carrying articles is required; stooping at times to file or store articles.

Sensory Attention:
Good communication skills required to communicate effectively with customers, exercise a high degree of tact and poise, and occasionally overcoming language barriers; talking and hearing essential in giving and receiving information regarding admitting process. Comprehensive ability required understanding and applying principles and procedures of marketing.
Mental Stress:
Moderate discomfort does occur when census decreases. Must be able to work under a certain amount of stress. Family and resident concerns can cause discomfort. Must have compassion and ability to properly handle emotional situations.
