$1800.00•$2700.00 POSITION SUMMARY•RN Case Manager
POSITION DUTIES
•Coordinate and oversee patients' interdisciplinary care plans, assess clinical and psychosocial needs, ensure appropriate resource utilization, facilitates discharge planning, and act as a liaison between patients, families, and the healthcare team to support optimal recovery outcomes.
MINIMUM REQUIRED QUALIFICATIONS
•AR/compact RN license Minimum of two-year nursing degree BLS 2 years recent acute hospital case management experience
PREFERRED QUALIFICATIONS
•Acute inpatient rehab experience Previous experience in clinical liaison, marketing, or healthcare sales
LENGTH OF ASSIGNMENT
•13 weeks
SHIFT / HOURS PER WEEK
•8a-5p with scheduled weekend coverage SYSTEMS•Meditech
