Immediate Hire _ The RN Case Manager will be a vital part of the company's growth, working closely with physicians and interdisciplinary team (IDT) members specializing in veteran hospice care. The role emphasizes patient care as a top priority, with a focus on continuous improvement, teamwork, and recognizing outstanding work. Work with management that care about their staff as much as they care about their patients. Preference for prior hospice experience and available to start immediately with experience in managing full case load of 15-18 patients.
Key Requirements:
Strong Clinical Skills:
Essential due to the complexity and chronic nature of the patients.
Experience:
At least one year of experience as a registered nurse.
License:
Unrestricted RN license in the state of Arizona.
Work Hours:
Full-time and part-time opportunities with assigned on-call responsibilities as needed. Responsibilities Provide skilled nursing services in accordance with physician orders and the plan of care in a patient's home. Oversee and coordinate patient care services. Participate in quality improvement activities to ensure compliance with state and federal guidelines and professional standards. Perform observation, assessment, nursing diagnosis, counseling, care, and health teaching for hospice patients and their families. Address risk factors such as pathological grief and consider cultural and spiritual implications. Utilize both verbal and non-verbal communication patterns effectively.
