Location:
In-Home Client Visits (Verde Valley and surrounding areas) Position Summary The Private Duty Nurse (RN) is responsible for providing in-home medication oversight and support to clients receiving non-medical and care management services through Caring Hearts. This role focuses on ensuring medication safety, compliance, and coordination with providers, while supporting clients in maintaining independence and optimal health outcomes. This position combines clinical skill with a high-touch, concierge-style approach to care. Key Responsibilities Medication Management Prepare and maintain weekly/monthly medi-sets (pill organizers) in accordance with physician orders Perform medication reconciliation at each visit, identifying discrepancies or changes Monitor for medication adherence, side effects, and potential interactions Ensure medications are stored safely and appropriately Client Assessment & Oversight Conduct brief nursing assessments during visits (vitals as appropriate, condition changes) Identify and report changes in client condition to Care Manager and/or physician Support chronic disease management through medication compliance Coordination & Communication Communicate with pharmacies regarding refills, changes, and delivery coordination Collaborate with physicians, family members, and care team regarding medication updates Provide clear documentation and visit summaries following each encounter Education & Support Educate clients and families on medication purpose, dosing, and safety Reinforce physician instructions and care plans Provide guidance to caregivers regarding medication-related boundaries (no administration unless authorized) Documentation Requirements Complete timely and accurate visit notes per CHOS standards Maintain up-to-date medication lists Document any incidents, refusals, or concerns immediately Qualifications Required Active RN license in the state of Arizona Minimum 1-2 years clinical experience (home health, geriatrics, or primary care preferred) Strong understanding of medication administration, interactions, and safety Valid driver's license and reliable transportation Current CPR certification Preferred Experience in home care, care management, or private duty services Familiarity with polypharmacy and geriatric populations Experience with medication reconciliation and care coordination Skills & Competencies High attention to detail and accuracy Strong communication and interpersonal skills Ability to work independently in a home setting Professional boundaries and ethical practice Compassionate, client-centered approach Work Environment In-home client visits (independent field work) Flexible scheduling based on client needs Travel between client homes is required.
