There are opportunities to cover on-call and weekends with extra pay. We are dedicated to providing compassionate hospice care to those in need. Candidates with •hospice case manager experience• will be given strong preference. Responsibilities Provide compassionate, comprehensive hospice care to patients, visits to home or facility-homes, ensuring comfort and dignity in end-of-life situations. Partner closely with the interdisciplinary team for a seamless care experience, solid team communicaiton, and high level of patient satisfaction. Conduct thorough patient assessments and develop individualized care plans in collaboration with interdisciplinary teams. Monitor patient progress and adjust care plans as necessary, adhering to HIPAA regulations. Educate patients and families about care options, symptom management, and resources available. Facilitate discharge planning and coordinate with healthcare providers to ensure continuity of care. Maintain accurate medical records and documentation in compliance with medical terminology standards.
