Our stable, growing agency was founded by an RN owner right here in Arizona almost 30 years ago and provide exceptional end-of-life care in communities all across the state. This is a
FULL-TIME
position for the Globe/Miami AZ area. Full-time employees enjoy a comprehensive benefits package including medical/dental, LTD, life insurance, PTO, 401K. Pay is Salary, paid mileage. There are opportunities to cover on-call and weekends with extra pay. Low caseloads so you can spend time with patients and families We are dedicated to providing compassionate hospice care to those in need. Candidates with •hospice case manager experience• will be given strong preference, but we are willing to teach the right candidate what hospice is all about. Responsibilities Provide compassionate, comprehensive hospice care to patients, visits to home or facility-homes, ensuring comfort and dignity in end-of-life situations. Partner closely with the interdisciplinary team for a seamless care experience, solid team communication, and high level of patient satisfaction. Conduct thorough patient assessments and develop individualized care plans in collaboration with interdisciplinary teams. Monitor patient progress and adjust care plans as necessary, adhering to HIPAA regulations. Educate patients and families about care options, symptom management, and resources available. Facilitate discharge planning and coordinate with healthcare providers to ensure continuity of care. Maintain accurate medical records and documentation in compliance with medical terminology standards.
