Job Description

Make a Meaningful Difference in Patients' Lives We are seeking a compassionate and dedicated Hospice Nurse (On-Call) to join our team. In this role, you will provide exceptional end-of-life care to patients with terminal illnesses, helping ensure comfort, dignity, and quality of life during their final journey. You will work collaboratively with patients, families, and an interdisciplinary care team to deliver personalized care that addresses physical, emotional, and spiritual needs. Key Responsibilities Provide on-call nursing care and support for hospice patients in accordance with individualized care plans. Respond promptly to patient and family needs, offering compassionate guidance and support. Assess patient conditions, monitor symptoms, and implement appropriate interventions to promote comfort. Administer medications and treatments as prescribed, evaluating effectiveness and adjusting care as needed. Educate patients and families on pain management, symptom control, comfort measures, and end-of-life care options. Maintain accurate, timely, and compliant clinical documentation. Collaborate closely with physicians, social workers, chaplains, and other interdisciplinary team members. Provide crisis intervention and emotional support during difficult or unexpected situations. Advocate for patient comfort, dignity, and quality of life at every stage of care. Qualifications Current Registered Nurse (RN) license in Arizona . Minimum of 1-2 years of nursing experience ; hospice, palliative care, home health, or related experience preferred. Strong clinical assessment, critical thinking, and decision-making skills. Excellent communication and interpersonal abilities. Commitment to providing compassionate, patient-centered care. Flexibility to work an on-call schedule and respond to patient needs as required.
