About the
Role:
We are seeking a compassionate and highly organized Registered Nurse Case Manager to join our hospice care team. This role is ideal for a nurse who values a holistic approach to end-of-life care , thrives in a team-oriented environment , and is dedicated to providing comfort, dignity, and support to patients and their families. Bilingual (English/Spanish) required.
Key Responsibilities:
Manage a caseload of hospice patients, coordinating care plans and services. Conduct comprehensive assessments and develop individualized care plans. Provide skilled nursing care, symptom management, and emotional support. Collaborate with interdisciplinary team members including physicians, social workers, chaplains, and aides. Educate patients and families on disease progression, medications, and care techniques. Maintain accurate documentation and ensure compliance with hospice regulations. Participate in on-call rotation as needed.
Qualifications:
Current RN license in Arizona. Minimum of 1 year of nursing experience; hospice or palliative care experience preferred. Strong organizational and time management skills. Excellent communication and interpersonal abilities. Ability to work independently and as part of a collaborative team. Commitment to compassionate, patient-centered care. Spanish-speaking required
