We are seeking a compassionate, reliable, and skilled Licensed Practical Nurse (LPN) to provide high-quality nursing care to patients in home and community settings in the

EAST VALLEY

. The Home Health LPN works under the supervision of a Registered Nurse (RN) and follows individualized care plans to promote patient health, recovery, safety, and independence. This role is ideal for nurses who are comfortable working independently, value patient-centered care, and enjoy the flexibility of home health nursing.

Key Responsibilities:

Provide skilled nursing care in patients' homes in accordance with physician orders and RN-developed care plans Monitor, assess, and document patient conditions, including vital signs, symptoms, and responses to medications or treatments Administer medications and treatments as prescribed Educate patients and caregivers on disease processes, medications, and self-care techniques Communicate effectively with patients, families, and the interdisciplinary care team Maintain accurate, timely documentation in compliance with agency, state, and regulatory requirements Report changes in patient condition promptly to the supervising RN or physician Ensure patient safety and follow infection control standards at all times Travel to patient homes within the assigned service area

Qualifications:

Active LPN license in the state of Arizona

BLS / CPR

certification Minimum 1 year of nursing experience preferred (home health experience a plus) Valid Arizona driver's license and reliable transportation Arizona Fingerprint Clearance Card Current TB test Must pass a background check Authorized to work in the United States Experience with Kinnser (preferred) Knowledge of Electronic Visit Verification (EVV) is a plus Strong organizational, communication, and time-management skills Ability to work independently with minimal supervision

Working Conditions:

Frequent travel to patient homes Exposure to various home environments and potential infectious diseases Flexible scheduling; evening or weekend availability may be required Ability to manage patient behaviors professionally and safely Typical visit length: 1-2 hours per patient Why Join Us?
