Summit Healthcare Services is currently seeking PRN RN's to support Start of Care visits and ongoing case management throughout the East, Central, and West Valley. Key Responsibilities Complete Start of Care assessments and develop individualized care plans Perform routine skilled nursing visits within the community Collaborate with the interdisciplinary team to ensure seamless care coordination Educate patients and caregivers for long-term success Maintain accurate and compliant OASIS-E documentation Qualifications Active Arizona RN License (or Compact License) Minimum 1 year of Home Health experience required OASIS-E documentation proficiency required AZ Level 1 Fingerprint Clearance Card + CPR certification
