Join our compassionate healthcare team as a Registered Nurse with Hospice Experience PRN in Phoenix West Valley! This dynamic role offers the opportunity to deliver exceptional patient-centered care in a supportive environment. As a vital member of our hospice team, you will provide specialized nursing services to patients facing life-limiting illnesses, ensuring comfort, dignity, and quality of life. This PRN position allows flexibility while making a meaningful difference in patients' final stages of life. Duties Conduct comprehensive patient assessments, including vital signs, physiological evaluations, and symptom management to develop personalized care plans. Provide direct patient care in-home settings, ensuring adherence to best practices and safety protocols. Monitor and document patient progress accurately within medical records while maintaining strict compliance with HIPAA regulations. Educate patients and families about disease processes, symptom control, medication management, and end-of-life care options with clarity and compassion. Perform diagnostic evaluations and coordinate with physicians for timely interventions in complex cases involving infectious diseases or behavioral health needs. Participate in by-weekly staff meetings Skills Demonstrated experience in critical care environments such as Level I Trauma Centers or Level II Trauma Centers, ICU units, or emergency departments. Strong knowledge of hospital medicine practices including medical-surgical nursing, acute care procedures, and inpatient/outpatient workflows. Proficiency in medical terminology, physiology knowledge, and diagnostic evaluation techniques essential for comprehensive patient assessment. Skilled in managing diverse patient populations including geriatrics, and behavioral health patients within hospital or outpatient settings. Expertise in case management activities such as utilization review, discharge planning, intake coordination, and post-acute care planning. Familiarity with HIPAA compliance standards and medical records documentation to ensure confidentiality and accuracy. Ability to adapt quickly to fast-paced environments like emergency rooms or trauma centers while maintaining high-quality patient service.
