Overview:
We are seeking a dedicated Registered Nurse Case Manager to join our healthcare team. The ideal candidate will have experience in managed care, primary care, or clinical settings, and possess a strong understanding of patient care and utilization management.Duties:
- Coordinate patient care plans and provide nursing support
- Conduct assessments to determine patient needs and develop individualized care plans
- Collaborate with healthcare providers to ensure quality patient outcomes
- Monitor patient progress and update medical records accordingly
- Educate patients and families on treatment plans and self-care techniques
- Adhere to HIPAA regulations and maintain patient confidentiality
- Utilize critical thinking skills in assessing and managing patient conditions
Skills:
- Current RN license with experience in case management
- Proficiency in utilizing electronic medical records systems
- Strong communication and interpersonal skills
