RN Care Coordinator

Position Summary:

Responsible for performing care coordination of assigned patients to ensure all care services are identified, scheduled, and delivered in a timely and appropriate manner. Performs care transitions based on appointments, testing, and referrals and coordinates with other facilities to obtain patient's medical information prior to scheduled appointments. Communicates frequently with patients, family members, and other departments to provide patient care that is appropriate to patient's condition and needs. Performs other customer service duties, including responding to patient complaints and providing instructions about procedures.

Required Qualifications:

Bachelor's Degree in Nursing preferred. 1+ year of previous patient care experience in a primary care, acute care, home health, long-term care, or related healthcare setting required. Care coordination experience preferred. Current and valid State of Arizona Registered Nurse license. Current cardiopulmonary resuscitation (CPR) certification or ability to obtain the certificate within 30 days of hire. Exceptional communication, interpersonal, and customer service skills.

$74,944.00 - $104,944.00 per year
