Job Description
Overview We are seeking a compassionate and experienced Hospice Registered Nurse (RN) to join our team on a PRN/as-needed basis. The Hospice RN provides skilled nursing care to hospice patients in accordance with the patient's plan of care, physician orders, and hospice philosophy, with a focus on comfort, dignity, and quality of life. Responsibilities Provide comprehensive nursing assessments for hospice patients in home, facility, or inpatient settings Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team Administer medications, treatments, and symptom management interventions Monitor patient condition and communicate changes to physicians and care team members Provide education and emotional support to patients and families regarding disease progression, end-of-life care, and comfort measures Respond to urgent patient needs and provide on-call support as assigned Complete accurate and timely clinical documentation in the electronic medical record Coordinate care with physicians, social workers, chaplains, aides, and other healthcare professionals Ensure compliance with hospice regulations, policies, and standards of care Qualifications Active Registered Nurse (RN) license in the state of employment Graduate of an accredited nursing program Minimum of 1 year of nursing experience required; hospice, home health, oncology, or palliative care experience preferred Current BLS/CPR certification Strong assessment, communication, and critical thinking skills Ability to work independently and manage changing priorities Reliable transportation and valid driver's license, if travel is required Schedule PRN/as-needed position Flexible scheduling, including weekends, evenings, holidays, and on-call shifts as needed Physical Requirements Ability to stand, walk, bend, lift, and assist patients as necessary Ability to travel between patient locations Preferred Qualities Compassionate bedside manner Strong interpersonal and family support skills Commitment to patient dignity and comfort-focused care
Pay:
$65.00 - $120.00 per hourBenefits:
