Job Description

Job Summary The Per Diem Hospice RN Case Manager is responsible for providing skilled nursing care, case management, coordination of services, and compassionate support to hospice patients and their families in accordance with the hospice philosophy of care. The RN Case Manager works collaboratively with the Interdisciplinary Team (IDT) to assess patient needs, develop individualized care plans, manage symptoms, provide patient and caregiver education, and ensure quality end-of-life care across various care settings including private homes, assisted living facilities, and skilled nursing facilities. The Per Diem Hospice RN Case Manager performs comprehensive nursing assessments, monitors patient condition, coordinates physician orders and medication management, and ensures compliance with hospice regulations, agency policies, CHAP standards, and CMS Conditions of Participation. This role requires strong clinical judgment, excellent communication skills, emotional resilience, and the ability to work independently while maintaining timely and accurate documentation within the electronic medical record system. The Per Diem Hospice RN Case Manager supports patients and families through the physical, emotional, psychosocial, and spiritual aspects of the end-of-life journey while promoting dignity, comfort, and quality of life. Flexibility in scheduling, responsiveness to patient needs, and participation in on-call or weekend rotation as assigned may be required.
Pay:
$45.00 - $50.00 per hour
