About Us Sapphire Hospice is a Medicare-certified hospice provider serving patients and families throughout the Phoenix metropolitan area. We are a mission-driven organization focused on delivering compassionate end-of-life care while building a strong clinical culture centered on accountability, teamwork, and service. Our current census is approximately 18-22 patients, creating an opportunity for the right clinical leader to make a meaningful impact on patient care, team development, and future growth. The Opportunity We are seeking an experienced Hospice RN to serve as Director of Nursing and RN Case Manager. This is a hands-on clinical leadership position with direct involvement in patient care, admissions, compliance, team leadership, and operational decision-making. You will work directly with ownership and play a key role in shaping the future of the organization. Responsibilities Oversee clinical operations and quality of patient care Manage RN case load and coordinate interdisciplinary team meetings Review admissions and clinical appropriateness Supervise and support nursing staff, CNAs, and clinical team members Ensure compliance with Medicare Conditions of Participation and state regulations Collaborate with the Medical Director on patient care planning Participate in on-call rotation as needed Help build a culture of accountability, teamwork, and excellent patient care Qualifications Active Arizona RN license required Minimum 2 years hospice experience required Prior Director of Nursing, Clinical Manager, Patient Care Manager, RN Supervisor, or leadership experience preferred Strong understanding of Medicare hospice regulations Excellent communication and organizational skills Ability to solve problems, take ownership, and follow through Valid driver's license and reliable transportation Not Yet a DON?
