Role Snapshot As an Intake Coordinator, you ensure every referral is evaluated quickly, accurately, and in full compliance—so eligible patients can start home health services without avoidable delays. You will manage end-to-end referral intake, confirm coverage, and keep all stakeholders aligned.
Key responsibilities include:
Process incoming referrals and validate completeness Verify insurance eligibility, secure initial authorizations, and record benefit specifics Review clinical documentation against Medicare, state, and payer requirements Enter and track referral activity in Homecare Homebase (HCHB) Partner with scheduling to support timely start-of-care visits Communicate clearly with referral sources, patients' families, and internal teams Required qualifications: 2+ years in a Medicare-certified home health agency 1+ year in home health intake/referrals/insurance verification 1+ year using HCHB (required) If you're ready to bring precision and ownership to intake, apply today. A Typical Workday Your morning begins by triaging new referral notifications and organizing priorities for the day. You move between payer portals, phone calls, and documentation review, keeping notes tight and audit-ready. In HCHB, you update statuses and timelines so everyone has a single source of truth. Midday often involves clarifying details with hospitals, physician offices, and families to prevent downstream delays. As the day progresses, you stay closely aligned with scheduling to keep start dates on track, then close out the afternoon by confirming next steps, documenting outcomes, and leaving the queue clean for the next business day.
