You will manage end-to-end referral intake, confirm coverage, and keep all stakeholders aligned.

Process incoming referrals and validate completeness Verify insurance eligibility, secure initial authorizations, and record benefit specifics Review clinical documentation against Medicare, state, and payer requirements Enter and track referral activity in Homecare Homebase (HCHB) Partner with scheduling to support timely start-of-care visits Communicate clearly with referral sources, patients' families, and internal teams Required qualifications: 2+ years in a Medicare-certified home health agency 1+ year in home health intake/referrals/insurance verification 1+ year using HCHB (required) If you're ready to bring precision and ownership to intake, apply today.
