Full-Time Pay:
$83,700 - $86,789/year DOE About Sapphire Hospice Sapphire Hospice is a Medicare-certified hospice serving patients and families throughout Maricopa County. Our team is committed to providing compassionate, high-quality end-of-life care while supporting patients and families through every stage of their journey. Role Overview The RN Case Manager serves as the primary clinical coordinator for assigned hospice patients, providing direct nursing care, coordinating services, and ensuring patients receive exceptional care in homes, assisted living facilities, and other care settings. Responsibilities Complete initial and ongoing patient assessments Develop and update individualized plans of care Coordinate care with physicians, social workers, chaplains, aides, and other interdisciplinary team members Provide symptom management and medication oversight Educate and support patients, families, and caregivers Maintain accurate and timely documentation in compliance with hospice regulations Participate in IDG meetings and case conferences Participate in on-call rotation as assigned Qualifications Current Arizona RN license required Minimum 6 months hospice experience preferred Strong assessment, communication, and critical thinking skills Experience with electronic medical records/documentation systems Valid driver's license and reliable transportation What We Offer Competitive salary based on experience Mileage reimbursement Paid time off Supportive clinical leadership team Manageable caseloads and strong interdisciplinary support Opportunity for professional growth and advancement Service Area Maricopa County, Arizona If you are passionate about hospice care and want to make a meaningful difference in the lives of patients and families, we encourage you to apply.
