Full-Time Pay:

$83,700 - $86,789/year DOE About Sapphire Hospice Sapphire Hospice is a Medicare-certified hospice serving patients and families throughout Maricopa County. Our team is committed to providing compassionate, high-quality end-of-life care while supporting patients and families through every stage of their journey. Role Overview The RN Case Manager serves as the primary clinical coordinator for assigned hospice patients, providing direct nursing care, coordinating services, and ensuring patients receive exceptional care in homes, assisted living facilities, and other care settings.
