Job Description

It recognizes death as a normal part of life and engages not only the patient but also loved ones as it provides a team of specialists to care for people as they write that final chapter of life's journey.
SUMMARY A
hospice admission nurse evaluates and admits patients into end-of-life care. They conduct clinical assessments, explain hospice services to patients and families, complete intake paperwork, and collaborate with healthcare teams to create an individualized comfort-focused plan of care.
DUTIES & RESPONSIBILITIES
Patient Evaluation:
Review medical records and conduct in-home or hospital visits to verify terminal prognosis (life expectancy of 6 months or less) and assess the patient's immediate medical and emotional needs.
Education & Support:
Serve as a nurse educator by explaining hospice benefits, discussing disease progression, and supporting patients and families through a difficult transition.
Care Planning:
Collaborate with attending physicians and the interdisciplinary team (social workers, chaplains) to develop an initial, comfort-focused plan of care.
Documentation & Compliance:
Ensure all informed consents are signed, process necessary legal/medical paperwork, and document baseline data into the electronic medical record.
Symptom Management:
Administer preliminary treatments and medications to address acute pain or distress while on-site.
Resource Coordination:
Arrange for the delivery of essential medical equipment (e.g., hospital beds, oxygen) and order necessary medications before handing the case to the assigned case manager. Pay:
$80,000.00 - $90,000.00 per year
