Pay Range:

$2500 - $2600 weekly gross Requirements Minimum 2 years of RN Home Health experience required Homecare Homebase (HCHB) experience required Active RN license BLS Certification required Strong assessment, documentation, and patient education skills Ability to work independently and manage time effectively Responsibilities Provide skilled nursing care to patients in their homes according to physician orders and care plans Perform comprehensive patient assessments and develop individualized care plans Administer medications, wound care, treatments, and other nursing interventions Monitor patient progress and report any changes in condition to the physician and care team Accurately document all patient visits and care provided using Homecare Homebase (HCHB) Educate patients and families regarding disease management, medications, and home care needs Coordinate care with physicians, therapists, case managers, and interdisciplinary team members Ensure compliance with infection control standards and agency policies Promote patient comfort, safety, and independence in the home setting Required Documents to Submit Your Profile To begin the onboarding and profile submission process, please provide:
