Job Overview Join our dynamic healthcare team as a Home Health Admission Nurse, where your expertise will directly impact patient outcomes and enhance the quality of home-based care. In this vital role, you will be responsible for conducting comprehensive patient assessments, coordinating admission processes, and ensuring seamless transitions from hospital or clinic settings to home care. Your critical care experience, ICU background, and knowledge of complex medical conditions will empower you to deliver exceptional patient service and support recovery in the comfort of patients' homes. Duties Perform detailed patient assessments, including vital signs, physiology evaluation, and medical history review to determine eligibility for home health services. Coordinate admission procedures by reviewing medical records, discharge summaries, and diagnostic evaluations to develop personalized care plans. Collaborate with hospital staff, primary care providers, and case managers to facilitate smooth transitions from inpatient or outpatient settings to home care. Educate patients and families on treatment plans, medication management, infection control practices, and safety protocols aligned with HIPAA regulations. Conduct intake interviews and discharge planning to ensure all necessary services are arranged prior to patient admission. Document all clinical findings accurately in medical records and ensure compliance with healthcare standards and utilization management protocols. Communicate effectively with interdisciplinary teams to coordinate post-acute care, outpatient services, or clinic follow-ups as needed. Experience Proven critical care experience with a strong background in ICU, emergency medicine, or trauma centers (Level I or Level II Trauma Center preferred). Extensive hospital medicine experience across medical-surgical units, infectious disease care, or post-acute settings. Prior outpatient or clinic experience in geriatrics, behavioral health, pediatrics (PICU), or outpatient rehabilitation is highly valued. Demonstrated proficiency in patient assessment techniques, vital signs monitoring, and diagnostic evaluation processes. Familiarity with medical terminology, inpatient documentation standards, and discharge planning procedures. Knowledge of HIPAA compliance and medical records management is essential for maintaining patient confidentiality. Experience in utilization management, case management, or managed care environments enhances your ability to coordinate comprehensive patient services effectively.
