The ideal candidate will possess a strong commitment to patient care and have experience in various medical settings. As an LVN/RN, you will play a crucial role in providing high-quality nursing care, assisting with patient assessments, and collaborating with healthcare professionals to ensure optimal patient outcomes. Duties Provide direct patient care, including administering medications and treatments as prescribed. IV treatments are mandatory. Conduct patient assessments and document vital signs, medical history, and other relevant information. Assist in developing and implementing individualized care plans for patients. Collaborate with interdisciplinary teams to coordinate patient care and ensure continuity of services. Perform procedures such as IV insertion, catheterization, and wound care while adhering to infection control protocols. Educate patients and their families about health management, medication administration, and post-discharge instructions. Maintain accurate medical records and documentation in compliance with HIPAA regulations. Support patients with activities of daily living (ADLs) and promote their overall well-being. Participate in quality improvement initiatives and adhere to safety standards within the healthcare facility. Requirements Valid LVN/RN license in the state of practice. Experience in critical care, long-term care, assisted living, or similar environments is preferred. Familiarity with electronic medical records (EMR) systems such as Hospice MD and Synergy Home Health. Knowledge of
ICD-9/ICD-10
coding and medical terminology is essential. Strong clinical skills in areas such as IV infusion, triage, pain management, geriatrics, pediatrics, or emergency medicine are highly desirable. Ability to work effectively with individuals with developmental disabilities or dementia-related conditions is a plus. Excellent communication skills and the ability to work collaboratively within a team-oriented environment. Basic life support (BLS) certification is required; additional certifications related to specialty areas are beneficial.
