Unit Details:

Adult and geriatric patient population with DFU, pressure injury, VLU, and arterial ulcers. Assesses, stages, measures, and photographs wounds; interprets reports and communicates findings with the assigned physician. Assists with wound therapy and implements wound care and HBO protocols when clinically indicated. Functions as a case manager for wound care patients, including identifying home health needs, coordinating office visits and pre-procedure work-ups, supporting patient education and discharge planning, and fostering continuity of care between the wound care center and home, hospital, and home health settings. Clinic responsibilities include triage telephone calls, scheduling support, and coordination with the multidisciplinary team.
