As a key member of our home health services, you will assess patient needs, develop tailored care plans, and coordinate with multidisciplinary teams to ensure seamless post-acute and outpatient care. Duties Conduct comprehensive patient assessments including vital signs, physiological evaluations, and medical history reviews to determine individualized care needs. Deliver skilled nursing interventions such as wound care, medication management, IV therapy, and infection control in accordance with physician orders and best practices. Coordinate discharge planning and facilitate smooth transitions from hospital or clinic settings to home-based care by managing medical records and documentation. Educate patients and their families on disease management, medication adherence, safety protocols, and health maintenance strategies to foster independence. Collaborate with physicians, case managers, therapists, and other healthcare professionals to develop and modify personalized care plans aligned with patient goals. Monitor patient progress through ongoing evaluation of symptoms, response to treatments, and vital signs; adjust care plans as necessary for optimal outcomes. Ensure compliance with HIPAA regulations, medical terminology standards, and hospital policies while maintaining accurate documentation for all patient interactions. Qualifications Proven critical care experience with a strong background in ICU, emergency medicine, or Level I Trauma Center environments. Experience working within hospital medicine or post-acute care settings such as inpatient or outpatient clinics. Knowledge of infectious disease management, behavioral health considerations, geriatrics, pediatrics (PICU), and medical-surgical nursing. Familiarity with hospital experience procedures including intake assessments, diagnostic evaluation interpretation, utilization management, and case management processes. Strong understanding of physiology principles, vital signs monitoring, discharge planning procedures, and medical records documentation. Ability to perform thorough patient assessments in diverse settings while demonstrating excellent patient service skills. Knowledge of HIPAA compliance standards along with medical terminology proficiency for accurate communication and record keeping. Valid RN license with current state registration; additional certifications in managed care or emergency room procedures are advantageous. Excellent communication skills combined with a proactive approach to patient-centered care in both inpatient and outpatient environments.
