Day | 5x8‑hour shifts (08:00 AM•04:30 PM)
Guaranteed Hours:
40 hours/week Position Summary We are seeking a seasoned Case Manager RN for a 6‑week travel assignment in an acute care facility. The nurse will manage a caseload of up to 25 patients, coordinating care, discharge planning, and utilization review. Candidates must have at least 2 years of acute care case management/discharge planning experience and prior travel assignment experience. First‑time travelers will not be considered. Requirements Minimum 2+ years of acute care case management/discharge planning experience Must have hospital experience (behavioral health experience does not apply) Prior travel assignment experience required (first‑time travelers not accepted) Patient ratios: 1:25 (days), 1:45 (weekends) Proficiency with Epic EMR Must be able to independently arrange DME, SNF, and ambulance transport Certifications Active RN license (California state; pending accepted but must be active by start date) Current BLS certification (AHA required) Skills Case management/utilization review: admission criteria, discharge planning, care coordination, disease management, plan of care, prior authorizations Regulatory knowledge: CMS, Department of Health, DRGs, The Joint Commission standards Acute hospital specialties: ED, ICU, MS, surgical services Professional skills: family meetings, treatment plan development, knowledge of HMOs/IPAs, foster care/adoption, substance abuse treatment options, homelessness, mental health crisis, advance directives, POLST, protective services processes Responsibilities Assess patients and coordinate care plans Organize and participate in family meetings Manage discharge planning and utilization review Collaborate with interdisciplinary teams including social services and physical therapy Ensure compliance with CMS, DRGs, and The Joint Commission standards Document care accurately in Epic EMR Participate in weekend rotation as required Work Environment Acute care hospital setting Patient population: neonates, adolescents, adults, geriatrics
