  • $2,600 Position Overview A leading healthcare facility is seeking an experienced Registered Nurse (RN)
  • Case Manager to provide acute care case management and discharge planning services within a hospital setting.
The ideal candidate will coordinate patient care, optimize resource utilization, and ensure safe and efficient patient discharges. Key Responsibilities Manage acute care case management and discharge planning activities. Develop and coordinate patient care plans. Collaborate with interdisciplinary teams, patients, and families. Ensure appropriate utilization of healthcare resources. Facilitate safe, timely, and effective patient discharges. Maintain accurate documentation and comply with healthcare standards.
Required Qualifications Education:
Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)
Certifications & Licensure:
Active California RN License (pending accepted; must be active before start) BLS (AHA) Required Experience Minimum 2 years of Acute Care Case Management / Discharge Planning experience Recent hospital experience required (behavioral health experience not accepted) Must be a seasoned traveler (first-time travelers will not be considered) Additional Requirements Weekend coverage required (every other weekend) Patient ratios:
