CARE CERT OR WCON REQUIRED

Key Responsibilities:

Perform comprehensive wound, ostomy, and continence assessments and develop individualized care plans. Recommend appropriate medications, dressings, and treatments. Monitor, evaluate, and document patient progress and treatment effectiveness. Collaborate with interdisciplinary healthcare teams and refer to other services as needed. Provide patient, family, and staff education on wound prevention and management. Participate in hospital-wide quality improvement initiatives and data collection. Develop and deliver staff education programs and in-service training. Manage and track specialized wound care equipment and supplies.

Required Skills & Experience:

Minimum 2 years of Med/Surg experience. Wound and Colostomy care experience required.

Required Education:

Associate's Degree in Nursing (ASN, ADN) from an accredited program.

Required Certifications & Licensure:

Active New York State RN License. Primary Source Verification from

NYSED. AHA BLS

Certification. Current certification in one of the following: Certified Wound Ostomy Continence Nurse (CWOCN)

• WOCNCB Certified Wound Ostomy Nurse (CWON)
• WOCNCB Certified Wound Care Nurse (CWCN)
• WOCNCB Certified Wound Specialist (CWS)
• ABWM Wound Care Certified (WCC)
