This salaried role is designed for an experienced RN who is passionate about delivering exceptional one-on-one care while helping shape clinical excellence across our organization. The primary focus of the role is providing hands-on nursing care and clinical oversight for an individual client, developing personalized care strategies that maximize outcomes, independence, and quality of life in the home setting. In addition to this primary clinical focus, you will support the broader nursing team through RN assessments, supervisory visits, care plan development, and clinical evaluations. Lead Nurses participate in a shared on-call rotation, offering remote clinical guidance and support to caregivers, clients, and nurses. Key Responsibilities:
Skilled Focus:
Perform thorough patient/client initial assessments and reassessments, evaluating the client's medical needs and developing the appropriate Plan of Care, supporting documentation/tools, and care strategies. Provide respite care for medically fragile patients, ensuring their comfort and well-being. Conduct 62-day supervisory visits to assess patient progress and ensure adherence to care plans. Participate in alternating after-hours on-call phone duty with other nursing staff to address urgent patient needs. Manage electronic medical records (EMRs) to maintain accurate and up-to-date patient information. Fulfill LVN/RN duties as needed, serving as a role model and mentor for the nursing team.
Case Management:
Oversee nursing case management to optimize patient outcomes and promote efficient care delivery. Coordinate and lead the care staff to ensure smooth operations and efficient patient care. Act as a liaison between doctors and clients, facilitating effective communication and collaboration. Deliver training sessions for staff and clients, promoting best practices and ensuring compliance with healthcare standards. Handle any other duties assigned by management to support the smooth functioning of our healthcare facility.
Requirements :
Hold a current California Registered Nurse license or LVN license in good standing. Maintain current Basic Life Support (BLS) certification. Possess a clean driving record and have a reliable form of transportation. Successfully pass a comprehensive background check, physical and TB screening to ensure patient safety and welfare.
