Position Overview:
Kings View is seeking a Licensed Vocational Nurse (LVN) to provide direct nursing and personal care support within Medical Day Services. Working under the supervision of a Registered Nurse (RN), this role is responsible for medication administration, monitoring health status, and supporting Restricted Health Care Plans (RHCPs) while promoting the overall well-being of program participants.
Key Responsibilities:
Clinical Care & Health Monitoring:
Monitor and assess individuals' health status and report changes to RN Administer medications, treatments, and PRN medications per physician orders Measure and record vital signs (BP, pulse, temperature, etc.) Assist with RHCP implementation and physician coordination Maintain accurate medical documentation
Personal Care & Support:
Assist with daily living activities (bathing, grooming, toileting, mobility) Support individuals in maintaining independence and dignity Foster positive relationships with participants and care staff
Compliance & Safety:
Ensure compliance with HIPAA, Title 17, Title 22, and HCBS regulations Maintain proper storage and tracking of medications Serve as a Mandated Reporter and complete incident reports as required Maintain a safe, clean, and organized work environment
Program Support:
Assist with intake reviews and RHCP development Identify training needs for staff and communicate to leadership Support program operations and continuous improvement efforts
General Duties:
Maintain professionalism and confidentiality Collaborate with multidisciplinary teams Perform additional duties as assigned
What You'll Bring:
Required Qualifications:
High School Diploma or equivalent Graduate of an accredited nursing program Valid LVN license in the State of California Current CPR and First Aid certification Valid California Driver's License (Class C)
Required Experience:
One (1) year of experience in healthcare or community setting
Required Skills:
Strong knowledge of medication administration and nursing care Ability to assess health needs and respond appropriately Strong communication and documentation skills Ability to work with diverse populations and multidisciplinary teams
Preferred Qualifications:
Experience working with adults or seniors EHR system experience Bilingual English/Spanish preferred
