Duties Conduct thorough patient assessments including vital signs, physical examinations, and physiological evaluations to develop personalized care plans. Administer medications via injections or IV infusions following aseptic technique and safety protocols. Manage complex medical needs such as wound suturing, ostomy care, catheterization, and ventilator management in home settings. Coordinate with interdisciplinary teams to ensure seamless discharge planning and ongoing case management for patients with chronic conditions or post-acute needs. Educate patients and families on disease management, medication administration, nutrition, and safety practices tailored to individual health goals. Maintain accurate medical documentation using EMR (Electronic Medical Record) systems like Epic or Cerner while ensuring compliance with HIPAA regulations. Provide emergency response and triage for urgent health concerns while utilizing critical care experience when necessary. Skills Extensive nursing experience across diverse settings including hospital medicine, ICU (Intensive Care Unit), emergency medicine, long-term care facilities, hospice & palliative medicine, and home & community care environments. Proficiency in EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks for accurate documentation and patient tracking. Strong knowledge of medical terminology, ICD coding (ICD-9/ICD-10), CPT coding, laboratory procedures, and diagnostic evaluation techniques. Skilled in performing advanced procedures including IV insertion, phlebotomy, tube feeding management, sterile processing techniques, and ostomy care. Experience working with individuals with disabilities or developmental disabilities as well as geriatric populations requiring memory care or dementia support. Ability to manage acute pain cases effectively while applying trauma medicine principles from Level I/II trauma centers or critical care transport experience. Familiarity with Workers' Compensation law related to occupational health cases and infectious disease control protocols. Capable of providing specialized care such as botulinum toxin treatment or esthetic laser treatments when applicable; adept at teaching hospital protocols and outpatient procedures. Excellent patient assessment skills combined with physiology knowledge to evaluate complex cases involving cardiac catheterization or gastrostomy tube management.
