This role focuses on supporting patients and families through serious illness, goals-of-care discussions, end-of-life planning, and symptom management within an acute care hospital setting. The Palliative Care RN Coordinator serves as a clinical resource and patient advocate, collaborating with physicians, nurse practitioners, patients, and families to ensure compassionate, patient-centered care.

Week Shift:
Days (7:30 AM-4:00 PM or 8:00 AM-4:30 PM) Responsibilities Conduct comprehensive patient assessments and review medical records. Manage a daily caseload of new and follow-up palliative care consultations. Facilitate goals-of-care conversations with patients and families. Provide education regarding disease progression, treatment options, advance directives, and POLST documentation. Coordinate and participate in family meetings. Support end-of-life care planning and symptom management discussions. Attend interdisciplinary rounds and collaborate with the palliative care team. Develop and manage daily patient lists and care plans. Serve as a liaison between patients, families, physicians, and healthcare staff. Qualifications Required Active Registered Nurse (RN) license. Current BLS Certification. Minimum 2 years of recent experience in palliative care and/or hospice nursing. Strong acute care experience. Excellent clinical assessment, communication, and critical-thinking skills. Ability to work independently while collaborating with an interdisciplinary team. Preferred ELNEC (End-of-Life Nursing Education Consortium) Certification. ICU experience. Oncology experience. Hospice nursing experience. Schedule & Requirements 8-hour day shifts. Every other weekend rotation required. Holiday rotation with core staff. No call requirement. No floating assignment; however, nurses will consult with patients throughout multiple hospital units. Business casual attire with comfortable shoes and white lab jacket.
Pay:
$60.00 - $66.00 per hour
