We are seeking a compassionate and strategic Home Health Manager to lead our skilled home healthcare operations. In this vital role, you will orchestrate the delivery of outstanding, comprehensive clinical care directly in patients' homes, as well as oversee the quality of care provided within contracted nursing homes and board and care facilities. Blending clinical oversight with a passion for people leadership, you will guide and mentor a multidisciplinary team of home care nurses, physical therapists, and occupational therapists. Duties & Responsibilities Department Leadership & Operational Excellence Operational Direction:
Direct a high-performing home care department, ensuring all daily workflows, organizational policies, and clinical protocols are followed precisely.
Staff Development:
Recruit, train, mentor, and coach all members of the home care team, actively fostering a supportive, healthy, and highly engaged workspace.
Care Plan Integrity:
Ensure home care nurses accurately deliver and document nursing interventions exactly as outlined in each patient's personalized care plan.
Assessment Quality:
Continually assess and elevate the effectiveness of home health nursing assessments and active team care coordination.
External Care Oversight:
Monitor, evaluate, and safeguard the quality of care delivered to patients residing within nursing homes and local board and care facilities.
Multidisciplinary Coordination:
Efficiently arrange, schedule, and supervise specialized home-based services, including nursing, physical therapy, and occupational therapy.
Complex Case Resolution:
Apply advanced clinical knowledge to effectively manage intricate nursing scenarios and tackle complex medical issues unique to frail elderly populations. other duties as assigned.
Required Qualifications Licensure:
Active, unencumbered Registered Nurse (RN) license in the state of California.
Clinical Knowledge:
Demonstrated nursing acumen necessary to treat a frail, elderly population and navigate complex geriatric clinical scenarios.
Core Competencies:
Strong organizational skills, exceptional communication, and a natural ability to foster a positive, collaborative team culture.
Desired Qualifications Education:
Bachelor's Degree in Nursing (BSN) or a relevant healthcare field is preferred (additional years of relevant clinical experience may be substituted).
Home Health Experience:
Minimum of two (2) years of dedicated nursing experience within a home health setting working with a frail or elderly population.
Leadership Background:
One (1) or more years of formal leadership, supervisory, or management experience in a Home Health environment.
Location and Work Type Work Type:
Full-Time, Mid-Senior level permanent position.
Work Setting:
Clinical leadership combining office-based administration and community-based home health oversight.
Schedule:
Consistent, stable daytime hours offering an excellent alternative to traditional, high-burnout hospital management roles.
Compensation, Benefits & Perks Base Salary Range:
$109,240 - $144,197 USD annually (Determined by experience and qualifications).
