The ideal candidate will provide high-quality nursing care to patients in their homes while promoting comfort, independence, and recovery.
Responsibilities:
Deliver skilled nursing care during home visits Monitor patient health and update care plans Educate patients and caregivers Coordinate with healthcare providers Maintain accurate patient documentation
Qualifications:
Active RN license CPR/BLS certification Strong clinical and communication skills Home health experience preferred Ability to travel to patient homes Knowledgeable in Data Soft Logic (DSL) EMR. If not, willing to be trained
