Case Manager RN Autonex Chico, CA Job Details Full-time $70 an hour 1 day ago Qualifications RN License Full Job Description Job Overview We are seeking a dedicated and compassionate Registered Nurse (RN) to join our dynamic case management team. In this vital role, you will coordinate comprehensive patient care plans, facilitate seamless transitions across various healthcare settings, and advocate for optimal health outcomes. Your expertise in acute and chronic care management, combined with your strong clinical background, will ensure patients receive personalized, efficient, and effective care. Duties Conduct thorough patient assessments, including vital signs, medical history review, and physiological evaluations to determine care needs. Manage cases across diverse clinical settings such as hospital medicine, outpatient clinics, post-acute care facilities, and emergency departments. Develop and implement individualized care plans that align with medical records, discharge planning protocols, and health regulation policies. Coordinate multidisciplinary teams to ensure timely diagnostic evaluations, appropriate interventions, and continuity of care. Oversee utilization management by reviewing medical necessity for services, authorizations, and managing patient intake processes. Facilitate communication with patients and families regarding treatment options, discharge instructions, and post-acute care arrangements. Maintain accurate documentation within electronic health records (EHR), ensuring compliance with HIPAA regulations and medical terminology standards. Collaborate with healthcare providers to optimize patient outcomes through effective case management strategies in critical care environments such as
ICU, PICU
(Pediatric Intensive Care Unit), or Level I Trauma Centers. Monitor patient progress by assessing vital signs and conducting ongoing evaluations to adjust care plans as needed. Support behavioral health initiatives and infectious disease care protocols when applicable to enhance holistic patient well-being. Qualifications Current RN licensure with active state registration required. Proven experience in critical care settings including ICU, emergency room, or Level II/Level I Trauma Centers. Strong background in hospital medicine, medical-surgical units, or outpatient clinics with inpatient experience preferred. Knowledge of health regulation policies, HIPAA compliance, and medical records management. Familiarity with electronic health records (EHR) systems and diagnostic evaluation procedures. Excellent patient assessment skills along with the ability to manage complex cases involving geriatrics, pediatrics (PICU), or behavioral health populations. Demonstrated ability to coordinate discharge planning and post-acute care transitions effectively. Strong understanding of physiology principles relevant to acute and chronic conditions across various specialties including infectious disease care and managed care environments. Exceptional communication skills for engaging with patients, families, and multidisciplinary teams to promote patient service excellence.
