We are looking for a per-diem to part-time Pediatric Hospice & Palliative Care RN to join our team of dedicated, compassionate caregivers. This unique role will support children as well as adult patients who are facing serious illness. Legacy Hospice and Palliative are is committed to excellence, compliance, and patient-centered care — providing comfort, dignity, and support to patients and families when they need it most. Our team is made up of dedicated professionals who take pride in their work and the meaningful impact they make every day. We are looking for the ideal Pediatric Hospice and Palliative Care RN that is confident, empathetic and has a strong back background in hospice care, and is comfortable providing care and educating the families and patients and providing emotional support during some of lifes' most fragile moments. The RN plays a key leadership role in overseeing and supporting the clinical field team to ensure the delivery of high-quality hospice care in accordance with Legacy's standards, CHAP accreditation, and state and federal regulations. This position includes direct field supervision, case conferencing, staff education, and coordination with the interdisciplinary team (IDT). Essential Responsibilities Assess the needs of pediatric hospice and palliative care patients and families and develop individualized plans of care in accordance with physician orders and the interdisciplinary care plan. Provide skilled nursing care with a focus on pain and symptom management, comfort, quality of life, and emotional support. Coordinate care across home, hospital, facility, and community settings to ensure smooth transitions and timely communication. Educate and support parents, guardians, siblings, and caregivers regarding disease progression, medications, equipment, symptom management, and what to expect. Respond compassionately to patient and family concerns, providing counseling, reassurance, and culturally sensitive support. Collaborate closely with the interdisciplinary team to communicate changes in condition, update care plans, and ensure coordinated support. Support admissions and smooth transitions from hospital, facility, or home to Hospice services. Perform nursing assessments, medication coordination, DME coordination, documentation, and follow-up care. Participate in interdisciplinary group meetings, case reviews, care planning, and quality improvement activities. Represent Legacy Hospice and Palliative Care with professionalism, compassion, in homes, hospitals, facilities, and community settings Key Responsibilities Supervise and support field nursing staff to ensure timely and appropriate care delivery. Conduct home visits as needed to assess patients and provide direct clinical oversight. Collaborate closely with physicians, social workers, chaplains, and other IDT members. Participate in case conferences and quality assurance meetings. Ensure compliance with all hospice policies, procedures, and documentation standards. Provide ongoing education and mentorship to RNs, LVNs, and CNAs in the field. Respond promptly to changes in patient conditions and ensure coordination of care. Maintain accurate and timely clinical documentation in the EMR system. Pediatric healthcare experience required. Pediatric hospice and pediatric palliative care experience required Experience with pain and symptom management, including infusion pumps. Excellent assessment, observation, critical thinking, time management, documentation, and communication skills. Qualifications Active California RN License (in good standing). Minimum of 2 years of hospice (supervisory experience preferred). Strong knowledge of Medicare hospice regulations and CHAP standards is a plus. Excellent communication, leadership, and clinical assessment skills. Valid California driver's license and reliable transportation. Ability to travel throughout the Inland Empire region.
