Radiant Hospice seeks a compassionate Hospice RN experienced with providing direct in-home care to patients approaching end of life. Hospice RN oversees 12-15 patients within their assigned territory and coordinates with the IDG team to ensure continuity of care. If you're looking for a job with supportive team culture while providing support and guidance to people who are approaching end of life this job is for you! The Hospice RN oversees the hospice patient's care and will collaborate with the other members of the patient's care team, attending physician, and patients and their families to coordinate quality care. RNs evaluate a patient's physical condition and develop care plans that address any of the patient's medical needs or needs for social and spiritual services. Apart from providing comfort to patients, RNs are adept at counseling and guiding patient's families as well. Minimum Qualifications:
Graduate of an accredited nursing program. Current California Registered Nurse license. At least one (1) year of hospice or palliative care experience Minimum of two (2) years of experience as a professional nurse within the last five (5) years. Experience in a case management role Expertise in pain management, symptom control, crisis intervention, and family support. Excellent demonstrated critical thinking skills and superior clinical judgment. Excellent computer skills. Excellent customer service skills. Reliable cell phone with local number and coverage. Valid driver's license, proof of acceptable automobile insurance coverage, and reliable personal transportation. Ability to work independently and manage time effectively. Ability to make accurate, and at times, quick judgments. Ability to supervise others appropriately. Ability to respond appropriately to crisis outside of a hospital setting. Acceptance of and adaptability to different social, racial, cultural, and religious modes. Why work at Radiant Hospice?
