Job Summary:
We are seeking a Registered Nurse (RN) Clinical Liaison to drive referral growth and support patient transitions from acute care settings to home infusion services. This field-based role combines clinical expertise with business development and relationship management, partnering closely with physicians, discharge planners, case managers, hospital staff, and payers to identify patients who may benefit from infusion therapy and coordinate seamless transitions to home-based care.
Job Responsibilities:
- Generate referrals and grow territory business through relationship development and account management
- Build and maintain strong partnerships with physicians, discharge planners, case managers, hospital staff, and healthcare facilities
- Identify appropriate patients for home infusion services and facilitate smooth care transitions from acute care settings
- Collaborate with healthcare teams to develop care plans and coordinate discharge planning efforts
- Educate referral sources on available infusion services and clinical capabilities
- Serve as a clinical resource and liaison between referral sources, patients, caregivers, and internal care teams
- Manage referral opportunities, account relationships, and territory activity
- Support patient onboarding and ensure a positive patient experience throughout the transition process
- Work collaboratively with clinical, pharmacy, and operations teams to achieve patient care and business objectives
Qualifications:
- Current and unrestricted Registered Nurse (RN) license required
- Minimum 2 years of infusion healthcare experience required
- Sales, business development, healthcare marketing, or referral development experience preferred
- Experience working within acute care, hospital, or discharge planning environments preferred
- Strong relationship-building, communication, and presentation skills
- Ability to manage a territory and work independently
- Strong organizational, problem-solving, and care coordination abilities
- Comfortable interacting with multiple stakeholders across healthcare systems About the
