We are looking for a home health RN who has experience with visiting patients in the field and completing OASIS documents. This would be an ideal position for an RN who is looking for an additional income opportunity while still holding down their regular job. Competitive pay rates based on experience, skill level.
PLEASE NOTE
This is NOT shift work. You will be assigned patients in a certain area and perform evaluations and/or skilled nursing visits in the patient's home. Dependable, insured transportation is a must for this position. •

Job Description Summary:
The Registered Nurse plans, organizes and directs Home Health care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. Essential Job Functions/Responsibilities Patient Care 1. Completes an initial assessment of patient to determine Home Health needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of nursing process. 3. Assesses and evaluates patient's status by: A. Writing and initiating Plan of Care B. Regularly re-evaluating patient/family/caregiver needs C. Revising the Plan of Care as necessary 4. Initiates the Plan of Care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 7. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician's Plan of Care. 8. Counsels the patient and family in meeting nursing and related needs. 9. Provides health care instructions to the patient as appropriate per assessment and plan. 10. Assists the patient with the activities of daily living and facilitates the patient's efforts toward self-sufficiency and optional comfort care. Position Qualifications 1. Graduate of an accredited school of nursing. One to two years of recent acute care experience in an institutional setting. 2. Current RN licensure in California, CPR certification and valid driver's license. 3. One year of Home Health care experience required. 4. Management experience not required. Responsible for supervising LVN's and Home Health aides. 5. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. 6. Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping bending, kneeling and/or crouching. Visual acuity and hearing to perform required nursing skills.
Pay:
$70.00 - $140.00 per hour
