Our interdisciplinary team promotes health, independence, and quality of life through nursing care, rehabilitation services, social services, therapeutic activities, nutrition support, and care coordination. Position Summary The Registered Nurse (RN) is responsible for providing comprehensive nursing services to CBAS participants in accordance with California Department of Aging (CDA), California Department of Public Health (CDPH), CBAS regulations, and New Start CBAS policies and procedures. The RN collaborates with the Muti-disciplinary team to assess participant needs, develop individualized plans of care, monitor health status, and promote optimal health outcomes. Essential Duties and ResponsibilitiesClinical Nursing Services Conduct initial and ongoing nursing assessments of participants. Develop, implement, and update individualized plans of care. Monitor participants' health conditions, vital signs, and medication compliance. Provide skilled nursing interventions within RN scope of practice. Identify changes in participant condition and initiate appropriate interventions. Communicate with physicians, caregivers, and healthcare providers regarding participant needs. Care Coordination Participate in interdisciplinary team meetings and case conferences. Collaborate with physicians, therapists, social workers, dietitians, and other healthcare professionals. Coordinate referrals and follow-up care as needed. Assist in transitions of care and discharge planning. Medication Management Review medication regimens and monitor for potential adverse effects. Educate participants and caregivers regarding medications and disease management. Maintain accurate medication records and documentation. Participant and Family Education Provide health education related to chronic disease management, wellness, nutrition, fall prevention, and self-care. Support participants and caregivers in understanding care plans and health goals. Documentation and Compliance Complete timely and accurate documentation in participant records. Maintain compliance with CBAS regulations, HIPAA requirements, and organizational policies. Assist with quality assurance and regulatory audits. Participate in emergency preparedness and infection control activities. Safety and Quality Improvement Monitor participant safety and report incidents according to policy. Participate in performance improvement and quality assurance initiatives. Maintain a safe, clean, and therapeutic environment. QualificationsRequired Current and unrestricted California Registered Nurse (RN) license. Current CPR/BLS certification. Minimum one (1) year of nursing experience in adult health, geriatric care, community health, home health, skilled nursing, or related healthcare setting. Knowledge of chronic disease management and geriatric care principles. Strong assessment, communication, and documentation skills. Ability to work effectively within an interdisciplinary team. Preferred Experience in Community-Based Adult Services (CBAS), Adult Day Health Care (ADHC), home health, case management, or managed care programs. Bilingual abilities (English/Spanish, English/Mandarin, or English/Korean) preferred. Experience working with diverse cultural populations.
