Job Description

Title:
RN Case Manager - Surgery / Specialty Care (Craniofacial)
Location:
Culver City, CA Pay:
$40-45/hr
Schedule:
Monday-Friday | 8:00 AM - 4:30 PM (No weekends)
Job Type:
Contract (3-6 months) with strong temp-to-hire potential

What You'll Be Doing:
  • Coordinate care for patients across craniofacial, gender care, and surgical specialties
  • Act as liaison between providers, specialists, and families
  • Manage patient journey from pre-op preparation through post-op follow-up
  • Review charts and ensure patients are fully prepared for treatment plans
  • Conduct ongoing follow-ups, ensuring continuity of care and positive outcomes
  • Collaborate in care planning with physicians, nurses, and interdisciplinary teams
What We're Looking For:
  • Active California RN license
  • 2+ years of case management or clinical experience
  • Strong organizational and communication skills
  • Ability to manage multiple patients while maintaining attention to detail
  • Comfortable working in a fast-paced, provider-driven environment Nice to Have (But Not Required):
  • Craniofacial or cleft care experience
  • Experience coordinating surgical care
  • Epic EMR experience
Bilingual Spanish Pay:
$40.00 - $45.00 per hour
