Duties and Responsibilities:

Utilizes the nursing process to assess, plan and implement and evaluate patient care Coordinates care of assigned residents, including the assessment of the resident's needs and development of a resident care plan Assess signs and symptoms indicating changes in patient's condition Collects, analyzes, and interprets information from health care members and documents and reports to charge RN and/or MD Oversee unit staff to ensure delivery of timely and quality care Other LVN duties as assigned

Requirements:

Active California LVN license Must have 2 years experience working as an LVN in a Skilled Nursing Facility Able to multi task Must be able to work in a fast paced environment
