El Cajon, CA Job Details Part-time From $50 an hour 5 hours ago Qualifications RN License IV insertion Case management Full Job Description Overview We are seeking a dedicated and compassionate Registered Nurse for our Home Health division. The ideal candidate will provide high-quality, patient-centered care to individuals in their homes, ensuring their health and well-being. This role requires a strong clinical background, excellent communication skills, and the ability to work independently while managing a diverse patient population. Duties Conduct comprehensive patient assessments and develop individualized care plans. Administer medications and treatments as prescribed, including IV infusion and injections. Monitor patients' vital signs and overall health status, documenting changes accurately in electronic health records (EHR). Provide education to patients and families regarding health management, medication administration, and home care techniques. Collaborate with interdisciplinary teams to coordinate care for patients with complex medical needs. Perform wound care, catheterization, tube feeding, and other nursing procedures as necessary. Ensure compliance with HIPAA regulations and maintain accurate medical documentation. Assist with pain management strategies and acute pain evaluations. Support patients with developmental disabilities or memory care needs through tailored interventions. Participate in discharge planning and case management activities to facilitate smooth transitions of care. Experience Candidates should possess the following experience or skills: Proficiency in using EMR systems . Experience in critical care settings, including ICU or Level I/II trauma centers a plus. Familiarity with various medical specialties such as geriatrics, pediatrics, hospice & palliative medicine, and behavioral health. Knowledge of ICD coding (ICD-9/ICD-10) and NCQA standards. Background in home care or community-based settings is preferred. Skills in phlebotomy, venipuncture, specimen collection/processing, and basic life support. Previous experience in medical-surgical nursing or emergency medicine is advantageous. Understanding of infection control practices and aseptic techniques. Ability to perform physical examinations and diagnostic evaluations effectively.
