About Us We are a growing mobile wound care practice dedicated to delivering compassionate, evidence-based treatment to patients wherever they call home. Our patients are seen across diverse settings — private residences, skilled nursing facilities (SNFs), board-and-care homes, and assisted living communities. Position Summary We are seeking a Registered Nurse or LVN with a passion for wound management and patient-centered care. The ideal candidate is reliable, compassionate, and able to work independently while maintaining strong communication and coordination with our interdisciplinary team. Key Responsibilities Provide wound assessment, treatment, and follow-up in home and facility settings Communicate and coordinate care plans with physicians, facility staff, and caregivers Maintain accurate and timely documentation in the EMR Collaborate with case managers and other healthcare professionals to ensure continuity of care Drive between patient locations within the assigned region Qualifications Current and active California RN license or California LVN license Reliable transportation and willingness to travel between patient locations Strong clinical assessment and bedside skills Excellent verbal and written communication abilities Comfortable using electronic medical record (EMR) systems New graduates are welcome - must be eager to learn, coachable, and committed to professional growth Preferred Qualifications Experience in wound care, home health, hospice, or post-acute settings (preferred but not required) Wound care certification (WCC, CWCN, CWS, or similar) a plus Why Join Us?
